Vetting help please!

As you might have seen from my signature, I have received my vetting and medical form through email from the lovely ladies at recruitment.
However, I am literally sitting here gawping at these pieces of paper feeling lost and confused at what is being shown.
I was given a little envelope when I completed my fitness which requires me to write some bits on the front yet doesn't provide much information
Has anyone been through this recently that could give aid? I'm sitting here scratching my head and I'm not getting anywhere fast!

Basically you should have a sheet on paper saying "Put XYZ in the small envelope and then send to recruitment at XYZ address", i believe it's your eye test results among with a couple of others. Fill out ALL the forms you have (should be 3 if i remember correctly), and then put them into a larger envelope. Once done, put the small envelope which will have on it "Occupational Health Unit" with the documents specified on the list enclosed and place into the bigger envelope containing the forms. Send off to Recruitment, and job done.
I believe that's the process you're talking about.
